Effects of Increased Step Width on Frontal Plane Joint Mechanics in Older Adults During Stair Descent

چکیده
INTRODUCTION The challenging nature and high incidence of trips and falls in the elderly during stair negotiation has led to numerous research studies investigating lower extremity biomechanics in older adults [1]. Stair descent has been shown to yield greater impact peak ground reaction force (GRF), and greater peak knee internal abduction moment compared to stair ascent and level-walking [2,3]. Changing the step width (SW) during stair walking could change the direction of the GRF vector relative to the lower extremity joints and, may alter frontal plane joint motion. The purpose of this study was to examine the effects of increased SW on GRF and lower limb joint motion in healthy older adults during stair descent.

Effects of increased step width on frontal plane knee biomechanics in healthy older adults during stair descent.
BACKGROUND Peak internal knee abduction moment is a common surrogate variable associated with medial compartment knee loading. Stair descent has been shown to yield a greater peak knee abduction moment compared to level-walking. Changes in step width (SW) may lead to changes in frontal plane lower extremity limb alignment in the frontal plane and alter peak knee abduction moment. The purpose of...
متن کاملInfluence of light handrail use on the biomechanics of stair negotiation in old age.
The high incidence of falls in older adults during stair negotiation suggests that this task is physically challenging and potentially dangerous. The present study aimed to examine the influence of light handrail use on the biomechanics of stair negotiation in old age. Thirteen older adults ascended and descended a purpose-built staircase at their self-selected speed: (i) unaided and (ii) with ...
متن کاملMeasuring femoral neck loads in older adults during stair ascent and descent
Fractures at the femoral neck play an important role in morbidity and mortality among older adults. Understanding the loading environment is a crucial factor for reducing the incidence of fracture at this site. In this study, the stresses on the femoral neck during stair ascent and descent were estimated. These femoral neck stresses were also compared to the peak loading from internal hip momen...
متن کاملLoading of the Lower Extremity and Low Back When Using Wedge Orthotics during Walking and Stair Negotiation
Wedge orthotics are commonly prescribed for patients with hyperpronation and/or low back pain to improve lower limb alignment and to reduce pain. The purpose of this study was to examine the kinetic effects of medial and lateral wedge orthotics during walking and stair negotiation. Twenty-two healthy young adults participated in the study. Each participant wore no wedge (W0) as a baseline and l...
متن کاملFrontal joint dynamics when initiating stair ascent from a walk versus a stand.
Ascending stairs is a challenging activity of daily living for many populations. Frontal plane joint dynamics are critical to understand the mechanisms involved in stair ascension as they contribute to both propulsion and medio-lateral stability. However, previous research is limited to understanding these dynamics while initiating stair ascent from a stand. We investigated if initiating stair ...
